I had heard an official stating they thought that ice had dislodged at launch, rather than the insulation material as others have reported. He said it looked like ice when it hit and broke apart. Ice would have been heavy enough to break off some of the fragile tiles on the left wing.

Amen to that. There are people in NASA who just love to play "games" and are less than truthful to say the least.
