"I guess what I am after is the earliest version of Bible that included the Old Testament. I am curious who put it together and how it became the standard."

This is a huge subject which you can easily research on the Internet. The Jewish Torah (most of what today we call the Old Testament) has been very carefully preserved from BCE days, letter by letter in Hebrew. The English standard (including both Old and New Testaments) for most Christians is the 1611 King James Version (KJV) but, if you want more readable text in contemporary Engilsh, the New International Version (NIV) is probably easiest to read.

Here are a couple of links to get you started on "who put it together and how it became the standard.":

is the earliest version of Bible that included the Old Testament
Old Testament, Christian name for the Hebrew Bible, which serves as the first division of the Christian Bible (see New Testament). The designations Old and New seem to have been adopted after c.A.D. 200 to distinguish the books of the Mosaic covenant and those of the new covenant in Christ. New Testament writers, however, simply call the Old Testament the Scriptures.
