Even within medical care we have improved medical procedures and better medications which are not directly related to the coverage that is the coverage can not overcome the limitations of the best available procedures at the time. And, of course, quality of the procedures is not directly related to the cost of the procedures. More americans exercise regularly. And finally, unless you are talking just about newly born you can not discard the effect of earlier years, wars, etc....
