According to icbl.org, the last time was Iraq1. I have no idea what "a significant chance" is in the eyes of a military wrt. location and time frame. In the end, the decision is up to the members of the very exclusive landmine users club.

That's correct. With ~70$mio/year, the US is the leading remover. I've no idea how this number is correlated per capita or in % related to military budget.
