After getting off a helicopter back at base - he noticed a grenade on the ground and picked it up.

It went off in his hand.

One of those weird military technicalities - he lost 3 limbs in service to his country, but it wasn't in combat.

My Dad's career covers all of this same ground - 3 Purple Hearts from rather serious combat wounds, but his 2 worst injuries (including the one that eventually killed him) came outside of combat (but still in uniform) and generated no awards.

He was injured by an American grenade that he picked up in a landing zone.
