As I see it, there were various options depending on how the cards fell. Thus I believe the neocon coup d'etat succeeded -- and with W in place. That was first prize -- to be able to run the country from behind the curtain, prescribe all the policy and also keep the doyen of the Establishment as the nominal head to take responsibility***. But I agree with you, the repeated "credible threats" to Airforce One make me feel that W was also a target. So, clearly, he must have been prepared to go along with with what he was told to do and therefore there was no reason to "take him out".

*** which is what we are debating. Thus, it didn't matter who actually "gave" the order so long as it APPEARED to come from W. "Cuckold" he was, but dead he was not.
