But where is the inefficiency?

A woman died in Santa Barbara ER because M. Jackson came into the same ER. While attending to Jackson, they forgot about her and she subsequently died.

Have you ever been to an ER in a big city? It took me 10 hours to be attended to and released after an ambulance rushed me to an LA ER after an accident. Its a big world outside of Hot Springs and things don't always work like they do there. Talk about something you know.
