When you talk about things could happen in 40 years, you should be really careful. For example, China could, at any time, to let the market decide the birth rate. That alone would more than enough to solve the so-called "demographic problem".

KyroL, it all depends on the ratio of productive people v. those in need of healthcare and care takers v. the country GDP.

Absolute numbers reflect nothing except the size of a this type of market and it's needs - like adult diapers for example and anti - depresants
