He also spelled out for her just some of the many other health problems that obese people face.

Surely she is aware that she is obese. If he had stuck to the health issues he might have been fine. The rest of what he said was gratuitously tasteless.

Imo, talking like this goes beyond reasonable professional discussion of health problems which may be related to obesity. This can be easily perceived as crossing the line between blunt talk -- and being downright nasty.
