Jim, RE: Big difference between MD's and PhD's is pay. Our system rewards mechanics over analytical researchers. Probably on the order of 3 to 1. On top of that it's a lot easier and stable to bill medicare and insurance companies than compete and write reseach grant proposals. It isn't a question of rewarding mechanics over researchers. It is more a question of private practice versus academics. A researcher isn't usually compensated as well as someone in private practice, but a researcher doesn't have to work as hard, either.
