Of course hospitals have duties.

The issues under discussion are whether their
duty is to promote breast feeding to
the exclusion of other alternatives or whether
it is a violation of some duty for hospitals to inform
patients as to such alternatives.

I don't think that "fiduciary duty" dictionary definitions do much to inform these issues.
