Our electric co-op did a test, straight gas vs 10% ethanol with all their meter readers. One million rural customers so they covered a lot of miles. The ethanol averaged 1-2% better mileage.

No factual information sorry, but your conjecture might be part of the answer. That seems likely [the credit card discount idea]. In NZ we now get annoying discounts via supermarket vouchers, worth just enough to be bothered hassling with.

I don't know what the effect is of ethanol addition on MPG.
