I don't think anyone claims that Al Qaeda is responsible for most of the American casualties. There have been quite a few US military admissions that the insurgency is largely Iraqi, not foreign.

Lately, AQ's suicide truck & car attacks have caused a greater share of the casualties. While most of the insurgency is Sunii/Baathist personnel, most of the spectacular suicide attacks have been done by foreigners. The US military has been consistent on saying that too.
