This is one area not addressed by Dems with plans. They think it needs to be free to be fair. Without a reasonably sized copayment however, (mind you not large enough to bankrupt someone or keep them from getting care on real illnesses) demand will skyrocket. Just human nature. Also, liberal groups (and many moderates) worry about some doctors exiting the system and only providing fee for service business to the rich with resultant system of bad substandard care for masses and better, elite care for the rich. Canada tried to solve this by forbidding private care but it did not work. My personal feeling is that this system will develop in US if we go that route - not something with which I am comfortable BTW.
