<Or do we strive to provide for those at the margins while letting the general level of wealth go up while the market drives medical costs down>
whatever it is your smoking, it is likely not good for you...

Could you and those who think like you have a vested financial interest in keeping the system pretty much as it is?

A system where Americans pay higher drug prices than the citizens of any other advanced nation for instance.

Personally I have very good coverage but see so many who are suffering severely from inadequate insurance or no insurance at all in the face of soaring medical costs.

This is typical of those I find credible. The answer to that is amazingly simple. they have the cash to jump the queue. OTOH You will be served eventually, unlike some other places. The problem of course with market driven is that many just just cannot pay the price.. Will that unsatisfied demand drive the market to produce more doctors ? Of course not. What's in it for them if the price comes down. Maybe someday though economies of scale will permit cheap and efficient healthcare.. maybe..

Our Canadian system is crappy for sure and I love to complain about it. OTOH our Canucky system is much like Churchill's view of democracy.. It sucks, but it sucks much less than the alternatives all things considered. Depends simply on the type of society you want.

FWIW. GM has $1.5K in medical expenses in every car. US companies bear these expenses while trying to compete globally. So who is paying the price? All of us.
