Underweight births often are the result of poor prenatal care, but that's not 100% on the medical system or medical payment system either.

Care isn't just medical services provided by doctors and nurses. Its the pregnant woman taking care of herself, and not having unhealthy habits.

if other countries count as live births those who can actually survive a few hours or days... seems to me they aren't giving a true reflection of the state of things.

The US is counting them as live births, in some other countries they (or some of them, the criteria vary) are not counted as live births.

A case can be made for either method of counting, but if you have multiple methods, than you need to adjust for the differences to make comparisons meaningful.
