An old friend on SI, Heinz Blasnik used to comment on this same topic (how additional debt increases result in progressively smaller GDP increases once yer in Ponzi finance mode). Lots of debt trapping going on, and lots of debt created simply to refinance old obligations, not used to invest per se. And we're in the 3rd inning of this process as so much remains hidden beneath the surface (ie, in level 3 assets on balance sheets of US companies).
