Actually, I don't know for sure... I've always been very careful not to!
You're meant to be able to speak to the presiding officer (the person who swaps the poll card for your actual ballot), tell them your address, and get your ballot paper... I've no idea if they want ID, however. I know they don't if you give them the card - you just tell them your address and they check it on the list.
It doesn't sound hugely secure, does it... but postal/absentee ballots aside, there just don't seem to be problems.

The electoral roll is kept very separate even from local tax rolls, although it gets cross-checked against all kinds of other documents later.
