What irritates me are these MDs that insist you call them
Doctor [first name]. I'll call you Dr. Jones, or Charley but I WILL NOT call you Dr. Charley. It's demeaning, to me.

That must be why it is considered inappropriate to address judges as just "Judge", instead of "Your Honor" or "Judge Smith."
