Because their numbers make them stepchildren. If everyone were buying his own insurance and state regs didn't get in the way, then you'd see some options.

I can't speak for anyone else. I can tell you that I live in a sparsely populated state where the numbers should be small. And I could, should I choose to do so, get 10 or more insurance companies on the phone from whom I could buy individual insurance plans. This would include BCBS, UHC, AETNA, CIGNA, Humana, and practically every other major health insurance company.

All of these companies have plans that do not compete with their group offerings. To claim that the market is small, or as someone else did -- that they couldn't find a plan in or around NYC is just silly.
