No additional costs are incurred. There is a difference in usage of the term "attorney" in Canada. In the U.S. I believe that the term generally applies to a lawyer who is paid for a service.

In Canada the person to whom you give power of attorney for personal care need not be a lawyer. The person appointed can be anyone 16 years of age or older (subject to some restrictions).

The power of attorney becomes effective when a doctor or evaluator determines that you are incapable of making a decision. This is also when an advanced directive becomes effective.

The simplest thing to do is to give someone your medical power of attorney. If, of course, you have someone who understands you well enough to know what decisions you would make for your self and that you can trust to make them for you. It's a really simple legal document.

The advantage of this approach is that this person can make decisions on the fly for you rather than your having to specify in advance and in writing exactly which procedures you do or do not want, instructions that would then be interpreted by strangers without any flexibility to adjust for circumstances on the ground.
