The busy doctor has real life experience with patients and treatments. He only knows what he sees and what he sees is only what he's already doing in his practice. If he doesn't have/take the time to find out what else is out there, he has no way to evaluate what is out there against what he's doing. Inertia is a strong force. So is the need to believe that what you're doing is good work. It's really hard to look outside and come to the realization and acknowledgment that you're not doing as much good as you thought you were.
