Profit margins for health insurers have been noted in several articles.

If 40% of private insurance coverage is self insured (no profit) the profit that could be medical services is that much smaller.

Of course there is also another segment of the expenses of insurers like advertising that does not go for medical care.

I think the most effective way to reduce medical costs is to have people change their lifestyle and to have some out of pocket expense attached to each and every medical service.
