We can distinguish between physical consequences and moral consequences, which most of your hypotheticals cross into. Cigarette and alcohol taxes are the classic example. They hurt people that society pays to fix or care for until they die.

The other sports you mention don't cause enough injuries to be a significant cost. Millions of people getting diabetes at younger and younger ages is a clear, present and quantifiable danger. A study came out just recently about actual obesity caused by soft drinks.
