end game ends up costing about the same whither you do or don't look after your health

Precisely. I know you and I are not always fully on the same page on healthcare (maybe because we tend to focus not on the same aspects? -g), but it's strange how many people think that prevention saves money. In fact, it probably costs more - you pay for it, you pay for screenings plus evaluation and treatment of findings.... and over the years, what you end up having is a larger older population - which is expensive. End of life care would still cost about the same per person - and even a very healthy individual will eventually become old and sick.

Prevention is wonderful, it improves and prolongs lives, and should be practiced - but it involves more costs, not savings.

I don't believe that politicians are stupid - they simply have a vested interest in seeing things the way they do... and in brainwashing people.
