This happens all the time, just not with Medicare -- docs will go out of network with some insurance, they just send those patients a letter.

Depending on the specialty, it takes a pretty bold doc to run off his Medicare patients as that's a lot walking out the door at one time.

A clinic I work with had a very large Medicaid clientele and dropped them a few years ago; collecting the claims just wasn't worth the hassle.
