"Fail" doesn't mean fails in everything in the worst possible way. A president failing can mean he's unpopular, and/or that he doesn't get his domestic policy objectives passed, or that when he does he and his party lose elections and at least some of them get reversed. Even in terms of foreign policy (where I don't think Rush was directing his comments), it can mean that he fails to implement certain ideas, not that he fails to achieve widely shared objectives. Obviously Rush doesn't want Iran to get nukes, or otherwise gain power and influence that they use in negative ways. He doesn't want terrorist to blow up a nuke in New York City, however much it might be considered a failure by the Obama administration. Just as if I wanted a bridge project to fail, I would mean that I don't want it to get approved and funded, not that I want it to fall down and kill 50 people.
