I'd say this had an even greater effect than the reasons cited in the article, and explains the low participation in part D. As seniors communicate this - I'd expect participation to become even lower."

True. According to the Kaiser Family Foundation study from 2007, the Part D donut hole (eliminated by Obamacare) was a driving force behind the switch to generics.

Coincidentally, it likely accounts for some of the decline in innovation, since generics are now a more profitable business than before.
