The market is twisted and warped by the involvement of the insurance companies, so standards will not be set by the market they will be set by the insurance companies. There is no open market for health care. I wish there was.

Why is that the case? You can sell a low cost economy car and a cadillac in the same state, and that hasn't occurred. Many plans now provided are better than others and people and employers still take them. When I was providing health insurance for my employees I did not take the least costly,nor the most costly, I took the one that best suited their needs and that I could afford.
