I know what you mean here, but actually what the US Constitution says, I'm told, or has been interpreted to mean by the courts is that: What powers are not specifically given to the Federal Govt. by the US Constitution are reserved to the states:

Romney Care is blessed in Mass. because it's Supreme Court has said that under the Mass. State Constitution it is legal--just as gay marriage is there.

But to throw a Monkey wrench into what I've just said every state's constitution might not be able to be interpreted to allow for a state universal health plan--even if it's legislators wanted it (without amending such).
