You can self insure, but your still paying part A anyway, and you probably paid taxes for part B for a big part of your lifetime.

As for contracting with a doc, sure you can do it if he doesn't accept Medicare at all, but even if doctors aren't taking new Medicare patients, if they take Medicare patients at all they are covered by Medicare rules for 65+ patients.
