You can self insure, but your still paying part A anyway, and you probably paid taxes for part B for a big part of your lifetime. That's the law.....you've paid all your working life..... And I don't know where your part B comment comes from/??? if they take Medicare patients at all they are covered by Medicare rules for 65+ patients. I told you that if they refuse assignment they can enter in a contract with you and charge you what they want......
