Why are you so hung up on 'insurance' ? You do realize that insurance is necessarily a 'profit' making endeavor, even if it is 'non-profit'.

But who cares about insurance except those providing it? I (or anybody) can simply go to an ER and get medical attention --- bleeding or not. They ( or I ) may pay, or may not. Their insurance (if they have any) will pay, or not. Does not that provide the medical attention when they need it? There will always be goats that don't ( or can't ) follow the rules. What does the system do with them.

But ERs are expensive --- maybe, maybe not. They do bill at a high rate but that doesn't mean they are expensive. What would you propose to have happen to the ER, close it down? Have it go to 5% utilization?

Sure, it's not efficient. It is an 'on demand' service, just like fire departments. If we could have fires occur on schedule fire departments would be much more efficient. Is that the course you would recommend to the ERs under your purview? Is that how you ran the ER services --- efficient but non-responsive?

I would guess not.
