OT - When we were both working, we used to have our healthcare costs taken out pretax. Now that we file long form I feel better increasing my annual gifts to the SPCA, Make a Wish, and a few other smaller ones.

Wouldn't take a chance now, but maybe after the wife retires, Healthcare.gov will still be in business.
Would have to figure the cost plus a supplemental policy like most in France do.

We'll stick where we are at least this year. Can't argue with the wife when she continues to work and it is her pension paying.
It is what she wants to do. It shall be done. lol!

My health insurance premium will actually be less on the new state exchange.
Currently pay $617.54/mo for BCBS insurance on myself.
Platinum BCBS plan on the state exchange is priced @ $582.79/mo for 2014.

I suspect like you, at some point the medicare age will be raised. I see continued grid lock for 4-5 years, so nothing will be done for awhile. Then I would suspect some type of sliding or tiered transfer to a higher medicare age threshold, like SS.
....If you were born before such and such a date you are eligible for medicare at age.....?
