I am fully aware of that. I happen to have just started working at a company that is in the process of upending the way medicine is done. A big problem is legislation. Oh, have you seen this? tricorder.xprize.org That is a key part. With that and even existing software, patients can be triaged pretty effectively. But, legislation will have to be passed so that a remote doc can look at the diagnosis and either sign off or replace it. The final step, where no doc is required will be a pitched battle. Remember how hard it was to allow a nurse practitioner to prescribe without a doc intermediary? The fight over diagnosis will be worse.
