Great questions Gene, Re << : How is sildenafil metabolized. What % is affected by first pass elimination? What are the metabolites and to what degree do they also serve as enzyme inhibitors? At what dose does Viagra begin to inhibit the remaining PDE enzymes? What is the half life of Viagra and what effect does age and medical status play in its metabolism? >>
Iwant to see BigKNY3 locate this one in his data bases!! I 'll give you my answers: those data IMHO are not available, ie few or nobody knows. This is why Viagra should undergo longer testing before it gets released to the masses.
Next, we, ( you a me, Zebra Cacaito MissLil ?), are going to have to correct line by line Big K's post. I'll start with 1st paragraph:
<<Although it accounts for only 2% of the total weight of the body, the brain receives the largest proportion of cardiac output (16-20% of the body's blood).>>
Correction #1: largest amount of cardiac output goes to the kidney, 25%; brain only gets 15%
Correction#2: "Cardiac out put" is not equivalent to "body's blood".
( a technicality, but nonetheless a very important distinction).
<<The initial distribution of drug will reflect the cardiac output including well-perfused organs such as the brain, heart, liver, and kidney>>
Correction#3; should read << the initial distribution of a drug will
" depend " on the cardiac output (ie drugs don't reflect the cardiac output, and thus you cannot take blood drug measurements as a reflection of the cardiac output ), as well as the " state of perfusion" of various organs>>.

so much for paragraph one BigKNY3. You need to be VERY precise with these things otherwise....body..kaput!!GG

Other paragraph corrections soon too follow I hope for eveerybody's information and edification!!
Back tonite.

Gene: Excellent rhetorical questions!

And I am sure they will all be answered for you and all physicians after the FDA review and prior to the market approval. How soon? ....Under the user fee guidelines, the FDA is required to respond to Pfizer 6 months after the NDA application was accepted. The Viagra NDA was submitted in September, 1997.

You never answered my question: Whom do you consider the most influential physicians in the field of ED?

BigKNY3

PS Thanks for the quiz, it was lots of fun! Do you agree with TA concerning the % of blood flow to the kidney?
