Having lived in one of the former slave states for nearly 25 years now, I doubt that the three problems you outline can be dealt with unless the former slave states are removed from the Union. Investing in people is not something that this country, as it currently exists, is going to do.

There are a number of things we can do to reduce homelessness. "Hospitalizing the insane and the drug addicted" is one of them, if it is legal. Another is to re-connect them with family members who will give them a home. It won't reduce the over-all population, but there should be a federal law against dumping homeless on other locales Homeless Residents Got One-Way Tickets Out of Town.
Some homeless live in RVs. Give them a place to park and hook up to sewage, and it will be like living in a mobile home which is actually mobile. It's estimated that 11% are vets. The VA has domiciliary units, and can build more.
