Knowingly filing a false report is a crime. Its done anyway, but that isn't the real issue. Reporting an adverse effect without knowing whether or not it was the result of the vaccine isn't considered false report, and in fact is specifically encouraged by the program.

Over a hundred million Americans have been fully vaccinated. Million more have had one dose. Bad things are going to happen to any group that large. Some of them, are obviously unrelated (for example if they get injured in an accident) but even dropping all such things your are going to have a lot of negative health incidents on such a large population.
