I am not sure, but I suspect there is a strong correlation between those who think covid is a mild disease (or fake) and how many people they encounter. This is related to the 6-degrees of Kevin Bacon game. en.wikipedia.org

Essentially, I suspect that whole rural communities rarely leave their bubble and this isolation conveys a kind of immunity on the community. What they witness is that those around them don't seem to get ill and that leads them to the conclusion that the whole thing is overblown.

It's not an unreasonable position to avoid vaccines in an area where infection is unlikely, at least for a while. It is prudent to see what side-effects and consequences might be reported before getting a vaccine. I think the emergence of the delta variant has changed the calculus of that tactic.
