Yes, I get how iron clad the CYA construct can be. As far as construction goes, it's been a while and they probably have instituted OSHA rules such that what you say is true. We always did it the best way we could figure out at the time, given the specific situation. When OSHA isn't around I'm sure it's the old way, no "may still have no choice but to follow" considered. And one always has a choice. Or maybe one never has a choice. Who knows.
