They are making you fill out this form, because your brokerage firm needs to make sure you have read the Characteristics of Risk pamphlet and protect themselves from a lawsuit if you lose your shirt.

Having said that, selling covered calls only requires level I approval, but you will need level III approval if you want to buy calls (much more risk in that you can completely lose your invested capital).

I won't let myself buy covered calls anymore - I had bought the April Intel 85s back in December for 3/4. The stock started to run up and I sold them for 1 1/2 a few weeks later. Of course the run continued and they went as high as $15!!! The moral of the story is that they expired worthless the 3rd Friday in April. :)
