Period inside or outside of the second quotation mark?

I personally put the periods and commas outside of the quotation marks. However, what about in the cases of question and exclamation marks? Also, what if the quotation using exclamation or question marks is in mid-sentence?

Example: She responded, "What in the world are you talking about?", in a sarcastic tone.

or

In a fit of anger she replied, "Leave me out of this!"

Note: In the latter example, since the sentence ended with the 2nd quotation mark, would I use a period outside of the quotation? I typically will not use a period to end this type of sentence.
