Regarding patients (or "adventurers") trading up for the 100mg dose, I have heard that the scarce supply of Viagra and similar cost of the two pills make it advantageous to cut the pill in half. The odd shape makes it difficult, but not impossible. Thus, you may not want to conclude that the lower dose which carries the safer profile is because of effectiveness in all users.
