As a project manager, consultant for Y2K remediation of mainframe code within the securities industry, the code tested was already fully tested before it was submitted to the big inter-related test for Y2K. No one submitted any code that wasn't fail safe, it was a very small % of actual code thats in use. This industry from what I see is far ahead of testing since its so critical for the public confidence and there was plenty of $ to fix the right (file expansion).
