Example of how of the PLRP could be implemented in a hospital environment.

Within the electronic information warehouse, the system would provide access to complete patient information including information from HIS applications, document images and medical images. The system automatically creates an electronic patient chart at the time of admission/registration. New information is added to the patient's chart from disparate systems such as lab reports and transcription information. This information may create new chart deficiencies or remove outstanding deficiencies. As these deficiencies are satisfied, the chart is completed and archived for permanent storage on optical or film media.

Healthcare providers implement formal policies for chart completion. Deficiency analysts are responsible for reviewing charts throughout the completion process. Deficiencies may be entered into a "follow-up" table in the system. If a deficiency has been satisfied by a user, the system would indicate automatically that the deficiency was satisfied.

Electronic Signature makes it easy for physicians to review and sign relevant documents. Physicians can return documents electronically if the information is incorrect or is not assigned properly. The system tracks incomplete or delinquent physician records and notifies the system if a physician's privileges have been suspended.
