You are correct. There is no free lunch, just an 80% discount with a high interest rate on the charges. And for this a $16.95 monthly levy is exacted. Very attractive to emergency rooms and the like, but not the patient. Emergency rooms are the treatment location of choice for the uninsured because they can walk away without paying and they can't be refused admittance-at least not in this state.
I don't know if there is an online provider base. Certainly seems one is needed if enough people sign on.

Re-reading the brochure, it does appear that if you call an 800 number after you receive your card, they will identify the available physicians in your area and it also looks like they make the appointment, too. If you have not been approved for a line of credit, you pay at the time of service. Not too popular with the credit-lacking, I'd bet. Usual disclaimers, IMHO do your own DD, for that's not what this is.
