This can be avoided by using limit orders which are lower than the ask price. I always use this strategy, but with most brokers, a limit order means more commission ! And of course it also means that you may or may not get your fill. In a fast market, it means you will NEVER get a fill.

I KNOW this happens all the time with me at schwab. But the pru orders go thru perfectly. It got to a point that I never bother with options in my schwab account....! Prefer paying a higher commission for a hassle free trade !
