Lazarus, In these days withheld assent is never formally used, at least in British realms. I expect the monarch does ask the PM questions along the line of, "is this a good idea?"

Because the monarch is not elected withheld assent would have to be about something extremely fundamental and important. It's the PM and legislators who stick their necks out.
