Pretty much like the mafia does it except the "vig" and the penalties are more harsh.

Its probably built in to the exchange rate offered.

That makes more sense.. But I've seen it where fees are required, or at least that's how it was represented. Either way, the "spread" is a fee, even if built into the exchange rate.
